DATA CONSENT FORM
To be read and signed by all Individuals providing Personal and Sensitive Data.
Each adult mentioned under the insurance policy must complete a separate consent
form as may be relevant. Parents/Guardians should sign this on behalf of minors
(under the age of 18 years).
Jubilee Life Insurance Ltd (JLIL/us/we) are the Data Processor and Controller of your
personal and sensitive data which is being/has been collected, either directly by us or
through the insurance intermediary (agent or broker), that you have appointed, for the
purpose of provision of insurance/Life services that either:
- You have purchased (existing contract holder); or
- You are seeking to purchase (seeking a quotation/to transfer Life benefits); or
- Have been offered through your employer (existing/new corporate member).
The data processed will be used for the following purposes:
- Provision of Insurance cover/Deposit Administration and eligible benefits and
services where applicable.
- For research analysis and product development.
- To advise you on various matters related to your insurance policy/Life account.
- As may be required to comply with the relevant laws in Kenya.
The categories of personal data you are being asked to consent to JLIL collecting,
processing, and using includes but is not limited to, your name, postal and residential
address, email address, telephone number, date of birth, national identity card number,
and personal identification number.
The categories of sensitive data that you are being asked to consent to JLIL collecting
and processing where relevant are, your health details, medical data, personal and
health details of your immediate family members, details of your employer, income level,
details and documentation to support source of funds legitimacy in some cases, and bank
details where authorisation to debit premium directly has been given.
In the provision of insurance and related services, JLIL will share your personal and
sensitive data where applicable with various parties including but not limited to:
- Third Party Information Technology service providers who provide the various
platforms that support our service provision.
- Third Party partners who provide medical services for the purpose of underwriting
the insurance cover such as hospitals, doctors, and other such service providers on
our Medicals Provider Panel.
- Third Party providers who facilitate administration of premium/Life remittances or
income/withdrawal payments such as banks, employers, and other such service
providers where relevant.
- Third party providers who facilitate reinsurance services to your insurance policy.
- Third party providers who facilitate information/document verification where
relevant.
- Government and Regulatory authorities as may be required under the relevant laws in
Kenya.
Where the personal and sensitive personal data is provided through an insurance
intermediary this consent serves to also authorise them to collect the data from you.
Your privacy matters to us. All Third-Party service providers as well as the insurance
intermediaries that we work with, are contractually obligated to keep your personal data
confidential subject to appropriate safeguards to prevent it from unauthorised
disclosure.
Your personal and sensitive personal data will be stored in accordance with the record
retention requirements applicable to JLIL as an insurer in Kenya and processed in line
with the applicable laws of Kenya.
You have the right to request access to, rectify, erase, and stop the processing of your
personal and sensitive personal data at any time, however this may disrupt the services
JLIL can provide to you or may stop us being able to assist you.
You have the right to file a complaint with us if you feel that we may have violated
your privacy rights in any way. These rights are more specifically described in the
